Home Health Care Licensed Practical Nurse For Weekends (LPN)

We are seeking an experienced and compassionate Home Health Registered Nurse (RN) to provide high-quality patient care in the Ft. Smith, Arkansas area and surrounding communities. This role focuses on delivering skilled nursing services in patients' homes, conducting comprehensive assessments, completing OASIS documentation, and supporting patients throughout their care journey. The ideal candidate will have prior home health experience and be comfortable working independently in the field.

Pay Rate: $51.00 - $53.00 per hour

On-Call Requirement: One weekday every other week

Weekend Requirement: One weekend per month

Provide skilled nursing care to patients in their homes according to physician orders and care plans.

Complete and accurately document OASIS assessments.

Develop, implement, and update individualized care plans.

Monitor patient progress and communicate changes in condition to the interdisciplinary team and physicians.

Maintain timely and accurate clinical documentation in accordance with regulatory and agency requirements.

Coordinate care with physicians, therapists, social workers, and other healthcare professionals.

Participate in on-call and weekend coverage as required.

Active Registered Nurse (RN) license in the applicable state.

Recent home health nursing experience required.

Ability to work independently and effectively manage a field-based caseload.

Valid driver's license, reliable transportation, and ability to travel throughout the assigned coverage area.

Knowledge of Medicare home health regulations and documentation requirements.

Experience working with diverse patient populations in a home care setting.
