Work Environment\n

This position allows for a remote work setting, offering flexibility while maintaining a structured work schedule. The standard work hours are Monday to Friday, from 8:00 AM to 4:30 PM. However, occasional evening or weekend hours may be necessary to accommodate client needs. You will need to be available for on-call situations and have the capability to modify your schedule to provide continuous care and support for clients. Our remote operation relies on effective communication and coordination with clients and their families to ensure seamless service delivery.

Key Responsibilities\n

• Conduct comprehensive assessments of seniors' psychosocial needs, incorporating emotional, mental, and social factors to develop effective intervention strategies.

• Collaborate with clients and their family members to create personalized care plans that align with their goals and preferences

• Provide counseling and emotional support during times of transition, helping seniors and families adapt to various life changes and challenges.

• Advocate for clients' rights by connecting them to community resources and ensuring they receive appropriate support and services.

• Engage with interdisciplinary teams, including medical professionals, caregivers, and social work colleagues, to coordinate and elevate care provided to clients.

• Conduct regular client follow-ups to monitor well-being, review living conditions, and address any emergent safety or health issues.

• Facilitate communication among family members, addressing conflicts and promoting collaborative care approaches.

• Educate clients and families about available community programs, support groups, and resources that boost social engagement and enrich their quality of life.

• Maintain accurate documentation in electronic health record (EHR) systems regarding client assessments, care plans, and interactions.

• Keep current with changes in social work practice, policies, and regulations to uphold ethical standards in all professional activities.

Qualifications\n

• A Master's degree in Social Work from an accredited institution.

• A current and valid state licensure as a Licensed Clinical Social Worker (LCSW).

Preferred Experience\n

While two years of experience in a social work role is required, preference will be given to candidates with a background in community health or geriatric care settings. Familiarity with health care regulations and knowledge of community resources specific to Georgia is also advantageous.

Compensation and Benefits\n
