Work Environment\n

This position allows for a flexible work setting, primarily remote, which facilitates your interaction with clients in their homes or through community outreach. You will work closely with an interdisciplinary team, including nurses, caregivers, and healthcare administrators, fostering collaboration to ensure optimal care coordination. The work hours are typically Monday to Friday, from 8:00 AM to 4:30 PM, with occasional evening or weekend hours based on client needs. On-call availability may be required for urgent situations that arise outside of regular hours.

Key Responsibilities\n

As a Licensed Clinical Social Worker, your responsibilities will include:

• Conducting comprehensive assessments of seniors’ psychosocial needs, analyzing their emotional, mental, and social circumstances.

• Working collaboratively with clients and their families to develop tailored care plans that address individual goals and preferences.

• Providing counseling and emotional support, assisting seniors in navigating life transitions, such as illness, loss, and other challenges.

• Maintaining accurate and up-to-date documentation of assessments, care plans, and progress notes, ensuring compliance with ethical and regulatory standards.

Qualifications\n

• A Bachelor’s degree in Social Work from an accredited institution.

• A valid, state-issued Clinical Social Work license (LCSW).

• A minimum of two years of experience in a social work capacity, ideally within a hospice or geriatric setting.

Preferred Experience\n

Experience in end-of-life care, hospice services, or working with dementia patients is highly beneficial. Familiarity with local community resources and support groups, as well as cultural competency in working with diverse populations, will enhance your effectiveness in this role. Candidates with additional certifications in geriatric social work or related areas will be given priority.

Compensation and Benefits\n
