Key Responsibilities\n

• Conduct comprehensive psychosocial assessments for individuals, families, and support groups, prioritizing care based on initial evaluations and client needs.

• Maintain effective communication with interdisciplinary teams through regular documentation, participation in meetings, and coordination of treatment plans, ensuring holistic care delivery.

• Provide case management support to patients, collaborating with local hospitals and community resources to ensure comprehensive and appropriate care.

• Complete thorough and timely documentation in compliance with departmental and professional standards, safeguarding patient confidentiality and care continuity.

• Utilize evidence-based methodologies such as Cognitive Behavioral Therapy (CBT) while integrating other therapeutic modalities to fulfill individualized service plans for clients.

Patient Population / Client Focus\n

Qualifications\n
