Key Responsibilities\n

• Conduct thorough ongoing assessments to tailor treatment plans that meet the diverse needs of your patients.

• Facilitate open communication with supervisors, direct care staff, families, and guardians to discuss each individual’s progress and treatment adjustments.

• Maintain a structured schedule of appointments, home visits, and monitoring activities, ensuring effective follow-up and support for your clients.

• Coordinate home visits and in-home observations, including chart reviews and staff interviews, to gain a comprehensive understanding of the patient’s environment and circumstances.

• Participate actively in Interdisciplinary Team Meetings, sharing insights and strategies to enhance patient care.
\n
• Complete accurate monthly and quarterly progress notes to document treatment efficacy and guide future care decisions.
\n
• Organize training sessions aimed at certifying staff in implementing treatment plans and ensuring adherence to documentation practices.
\n
• Provide essential follow-up support through interviews and home visits, aiming to empower clients in their ongoing mental health journeys.
\n
• Maintain accurate and accessible records that facilitate communication and collaboration among all stakeholders involved in a patient’s care.
\n
• Act as a liaison with the wider mental health community, advocating for both the program and patients’ developmental goals.

• Assist in developing innovative training materials to continuously improve program efficacy and individual treatment outcomes.

Patient Population / Client Focus\n

You will work with a diverse range of clients, including adults and youth, addressing various mental health challenges such as anxiety, depression, trauma, and relationship issues. Your role will be critical in fostering a safe and supportive environment where clients feel empowered to express their concerns and work towards their mental wellness goals.

Qualifications\n

• Possession of a Master’s or Doctoral degree in Counseling, Psychology, or a closely related field.

• Valid and unrestricted licensure as a Licensed Clinical Professional Counselor (LCPC) in Illinois.

• A minimum of two years of relevant counseling experience, preferably in a clinical setting.

• A strong grasp of legal and ethical standards pertaining to mental health practice.

• Demonstrated excellence in interpersonal and communication skills, facilitating meaningful rapport with clients.

• Proven ability to collaborate effectively within a multidisciplinary healthcare team.

• Proficiency in utilizing electronic medical records (EMR) software for managing patient information and documentation.

Compensation and Benefits\n

We offer a competitive salary range of USD 90,000 to 100,000 annually, commensurate with your experience and caseload.
