Work Environment

This position is onsite at our state-of-the-art rehabilitation facility, designed to enhance the delivery of physical therapy services. Key Responsibilities

• Conduct thorough assessments of patients' physical abilities, functional limitations, and rehabilitation potential, developing personalized treatment plans based on these evaluations.
• Provide one-on-one therapy to an average of 7 patients each day, guiding them through a variety of mobility and rehabilitation exercises designed to restore function and promote recovery.
• Maintain clear and supportive communication with patients and their families, ensuring they understand their treatment plans, progress, and any necessary modifications to their care.
• Document patient progress accurately in our electronic health record system, collaborating with physicians and the rest of the care team to ensure coordinated care.
• Educate patients on exercises and strategies that they can practice at home to continue their progress beyond therapy sessions.

Patient Population / Client Focus

The Rehabilitation Physical Therapist will work primarily with a diverse group of patients, including elderly individuals recovering from surgeries or injuries, stroke patients, and those with chronic pain or mobility issues. You will employ evidence-based practices tailored to meet the unique needs of each patient, empowering them to regain independence in their daily activities.

Qualifications

• Current licensure as a Physical Therapist in the State of New York.
• A Bachelor's degree in Physical Therapy; a Master's or Doctorate is preferred.
• A minimum of 2 years of clinical experience in a rehabilitative setting is required.
• Strong interpersonal and communication skills to effectively engage with patients and their families.
• Ability to work collaboratively within an interdisciplinary team of healthcare professionals.

Preferred Experience

Experience in a skilled nursing facility or rehabilitation center is highly desirable. Familiarity with various rehabilitation modalities and a commitment to ongoing professional development will be considered advantageous. Compensation and Benefits

The Rehabilitation Physical Therapist position offers a competitive salary ranging from $100,000 to $120,000 per year, commensurate with experience and qualifications.
