Work Environment

This role operates in a home care setting, allowing you to deliver physical therapy treatments in the comfort and familiarity of patients' homes. You will work Monday through Friday from 9:00 AM to 5:00 PM, maintaining a consistent schedule that promotes work-life balance. As part of a multidisciplinary team, you will collaborate with other healthcare professionals to provide a comprehensive approach to patient care.

Key Responsibilities

• Assess patients' physical capabilities through thorough evaluations and develop personalized treatment plans tailored to their specific needs.
• Administer physical therapy treatments and exercises, ensuring to adjust techniques based on individual progress and responses.
• Support and educate patients and their families on exercises, lifestyle changes, and home modifications that can enhance recovery and prevent further injuries.
• Manage a daily caseload of approximately seven patients, ensuring that all assessments and treatments are documented accurately and timely.
• Utilize advanced therapeutic modalities and equipment, while adhering to safety protocols to optimize patient care.
• Effectively communicate with patients, families, and interdisciplinary team members to ensure continuity of care and address any questions or concerns.
• Regularly review and adjust treatment plans based on patient progress and feedback, engaging in continuous assessment to achieve optimal outcomes.

Patient Population / Client Focus

The Physical Therapist role at Echo Rehabilitation Center primarily focuses on adult patients recovering from surgery, injury, or chronic conditions requiring rehabilitative measures for improved well-being. Qualifications

• Must possess a valid New York State Physical Therapist License.

• A minimum of two years of relevant experience providing physical therapy services, preferably in a home health or outpatient setting.

Preferred Experience

• Familiarity with electronic health records (EHR) and documentation standards in a home healthcare setting.
• Certification in specialized therapeutic modalities, such as manual therapy or aquatic therapy, is a plus.

Compensation and Benefits

The compensation for this role ranges from $100,000 to $120,000 annually, commensurate with experience and qualifications.
