Work Environment\n

This role is primarily remote, allowing for flexibility while ensuring comprehensive support to seniors in one of our designated hospice centers. The work hours are typically Monday through Friday, from 8:00 AM to 4:30 PM, but may occasionally require evening or weekend hours to meet client needs. Flexibility is essential, as social workers must adapt their schedules to accommodate clients' visits and potential on-call situations, addressing urgent concerns as they arise.

Key Responsibilities\n

Your responsibilities as a Licensed Master Social Worker will include, but are not limited to:

• Conducting thorough psychosocial assessments of seniors to understand their emotional, mental, and social needs.

• Collaboratively developing personalized care plans that address the unique goals and preferences of each client and their families.
\n
• Providing counseling and emotional support to clients and their family members, assisting them with life transitions such as terminal illnesses and grief.
\n
• Advocating for clients' rights and ensuring access to vital community resources and services to enhance their quality of life.
\n
• Working closely with an interdisciplinary team, including healthcare professionals and caregivers, to coordinate comprehensive care.
\n
• Conducting regular in-home visits to monitor client welfare, evaluate living conditions, and address any emerging concerns.
\n
• Facilitating communication among family members involved in the client's care, addressing conflicts and promoting family engagement.
\n
• Educating clients and families on available resources, support groups, and programs that can enhance social interactions and community involvement.
\n
• Maintaining accurate documentation of client assessments, care plans, progress notes, and other relevant interactions to ensure continuity of care.

• Staying informed about current practices and regulations related to social work, ensuring compliance with ethical and professional standards.

Patient Population / Client Focus\n

In this role, the focus will be on seniors receiving hospice care. You will work closely with clients who are facing life-limiting illnesses, providing support through emotional challenges, care planning, and connecting them with available resources. Building rapport and establishing trust with these clients and their families is essential to facilitate meaningful conversations and interventions.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution is required.

• Current and valid state licensure as a Licensed Master Social Worker (LMSW) is mandatory.

• A minimum of two years of relevant social work experience, particularly in hospice or geriatric settings, is preferred.

• Strong knowledge of social work principles, particularly as they relate to aging and end-of-life issues.

• Exceptional assessment, counseling, and problem-solving skills, with a demonstrated ability to empathize with clients and their families.

• Effective communication and interpersonal skills to collaborate with multidisciplinary teams.

• Proficient in electronic health record (EHR) systems and related computer applications.

Preferred Experience\n

Experience in home health or hospice settings will be highly regarded. Familiarity with community resources available for seniors, as well as knowledge of local healthcare regulations, would also be beneficial.

Compensation and Benefits\n

We offer a competitive salary range of $60,000 to $100,000 annually, commensurate with experience.
