Work Environment\n

This remote position provides you the flexibility to work from the comfort of your own home while engaging directly with clients and their families through virtual platforms. Although primarily remote, the position allows for necessary on-site visits based on client needs, and interaction with a cohesive team of healthcare professionals, including nurses and addiction specialists. You will be working regular hours from Monday to Friday, 8:00 AM to 4:30 PM, with occasional evenings or weekends as required to meet client demands.

Key Responsibilities\n

• Conduct comprehensive assessments of clients' psychosocial needs, focusing on emotional, mental, and social factors that affect their recovery and adjustment in a detox setting.

• Develop and tailor personalized care plans collaboratively with clients and their families to address specific goals related to recovery.

• Provide counseling and emotional support, helping clients navigate significant life transitions, such as detox and early sobriety.

• Act as an advocate for clients, ensuring they have access to community resources and services that facilitate their recovery journey.

• Maintain effective communication with interdisciplinary teams to coordinate comprehensive care, enhancing client outcomes and service delivery.

• Monitor client progress through regular check-ins, addressing any emerging concerns or safety issues proactively.

• Facilitate family dynamics discussions, resolving conflicts and promoting healthy communication among family members.

• Educate clients and families about available support systems, recovery resources, and local programs to foster engagement and promote recovery.

• Document assessments, care plans, and progress notes accurately in electronic health record (EHR) systems, ensuring compliance with best practices and ethical standards.

Patient Population / Client Focus\n

You will work primarily with individuals in a detox program, supporting clients facing substance use challenges in various stages of recovery. Additionally, the role involves working with seniors who may have complex support needs related to both aging and any concurrent addiction issues. Your ability to empathize and establish rapport will be crucial in fostering a safe environment for clients to express their needs and concerns.

Qualifications\n

• Master's degree in Social Work from an accredited institution.

• Current and valid state licensure as a Licensed Master Social Worker (LMSW).

• A minimum of two years of experience in a social work role, preferably within a detox or geriatric setting.

Compensation and Benefits\n
