This position is fully remote, allowing our LCSWs to maintain a healthy work-life balance while providing vital mental health services. You will collaborate with a skilled multidisciplinary team, including psychiatrists, psychologists, nurses, and case managers, ensuring continuity of care and a holistic approach to patient treatment.

Key Responsibilities\n

• Conduct thorough mental health assessments, integrating psychosocial evaluations, diagnostic interviews, and risk assessments to identify the needs of patients.

• Collaborate with patients to create individualized treatment plans that align with their specific needs, goals, and cultural backgrounds, ensuring a personalized approach to care.

• Utilize evidence-based therapeutic modalities, including cognitive-behavioral therapy (CBT), dialectical behavior therapy (DBT), and solution-focused brief therapy (SFBT), in individual, couple, family, and group therapy sessions.

• Provide crisis intervention services when needed, working alongside other healthcare professionals to safeguard the well-being of patients experiencing acute distress.

• Document clinical findings meticulously in accordance with best practices and regulatory guidelines, including progress notes, treatment plans, and discharge summaries.

• Empower patients and their families by offering psychoeducation tailored to managing mental health challenges effectively.

• Engage in regular collaboration with the healthcare team, participating in treatment meetings and care discussions to ensure comprehensive patient management.

• Conduct outcome assessments consistently to monitor treatment effectiveness, adjusting care plans as necessary to better meet patient needs.

• Stay updated on advancements in mental health research and practices, infusing new knowledge into your clinical approach to enhance patient care.

Patient Population / Client Focus\n

In this role, you will primarily support patients receiving hospice care, often navigating a range of emotional challenges associated with terminal illnesses. Qualifications\n

• A Master’s degree in Social Work is mandatory.

Compensation and Benefits\n
