Job Title: Family Nurse Practitioner (FNP)

Position Summary

The Family Nurse Practitioner (FNP) provides comprehensive primary care services to diverse patient populations, including adult and geriatric patients, in a collaborative and patient-centered environment. This role focuses on preventive care, chronic disease management, patient education, and delivering high-quality clinical care to support positive patient outcomes.

Responsibilities

Provide primary care services including preventive, acute, and chronic care management
Assess, diagnose, and treat a wide range of medical conditions
Manage chronic diseases and pain management treatment plans
Educate patients on wellness, disease prevention, and healthy lifestyle practices
Collaborate with physicians and multidisciplinary healthcare teams to ensure quality patient care
Maintain accurate and timely patient documentation and medical records
Monitor patient progress and adjust treatment plans as needed

Qualifications

Active Family Nurse Practitioner (FNP) license required
Active DEA license required
Previous experience in primary care or related clinical setting preferred
Strong clinical judgment and patient-centered approach to care
Excellent communication, interpersonal, and teamwork skills
Ability to work independently and collaboratively within a healthcare team

Compensation commensurate with experience
