Key Responsibilities\n

• Conduct thorough clinical evaluations and psychosocial assessments to inform treatment planning and interventions for patients.

• Facilitate and lead individual therapy sessions, psychoeducational groups, and process-oriented therapy groups to support patient development and emotional processing.
\n
• Ensure that discharge appointments are confirmed and clearly communicated to both patients and their loved ones, enhancing the continuity of care as they transition out of our services.

• Collaborate with referral sources, outpatient providers, and other stakeholders to ensure seamless transitions into different levels of care, addressing patient needs throughout their treatment journey.

• Manage crises effectively, utilizing therapeutic techniques to stabilize situations in both individual and group settings.
\n
• Participate in clinical outreach initiatives and program tours to promote our hospice services and advocate for patients needing support.

Patient Population / Client Focus\n

You will primarily work with patients facing terminal illnesses and their families, addressing the complex emotional and psychological needs associated with end-of-life care. This includes providing grief counseling, facilitating emotional expression, and tailored support designed to enhance the quality of life during difficult times. Your role is essential in helping patients navigate their feelings and ensuring they receive compassionate care that respects their wishes and dignity.

Qualifications\n

• Master’s degree in Social Work from an accredited institution.

• Current and valid licensure as a Clinical Social Worker (LCSW) in California.

• The annual salary range for this position is between USD 120,000 – 130,000 , dependent on experience and caseload.
