Key Responsibilities\n

• Directly engage with clients both in virtual sessions and in community settings, focusing on those experiencing significant mental health challenges.

• Conduct thorough client assessments, determining appropriate interventions and supports tailored to individual needs.

• Develop and implement comprehensive, integrated care plans that link clients with necessary resources and services, such as housing, vocational training, and medical assistance.

• Employ evidence-based practices for brief therapy sessions, utilizing techniques that are developmentally appropriate and maintain a focus on client empowerment.

• Facilitate both individual and group therapy sessions, as well as family counseling, aimed at fostering resilience and recovery.

• Provide crisis intervention support when necessary, ensuring immediate assistance is available to clients in distress.

• Maintain an active caseload while documenting client progress, treatment modalities, and outcomes in compliance with COHC standards.

• Monitor client engagement in treatment, assessing readiness for discharge when appropriate, and ensuring continuity of care.

• Participate proactively in team meetings and committees to refine our service delivery model and improve client outcomes.

• Respond to on-call needs as assigned, offering flexible crisis support outside of standard hours.
