Work Environment

As a Family Nurse Practitioner at Bright Health Medical Center, you will work onsite in a dynamic assisted living facility, ensuring that patients receive personalized care in a supportive, collaborative environment. Your work schedule will typically be from 9:00 a.m. to 6:00 p.m., with provisions for occasional evening clinics and participation in weekend community outreach events as needed. Expect to engage with an average of 40 hours of meaningful work each week.

Key Responsibilities

• Conduct comprehensive health assessments and screenings for diverse individuals and families, ensuring that each patient receives tailored care that acknowledges their unique health challenges.
• Diagnose and manage both acute and chronic medical conditions, incorporating preventive care and health promotion strategies to improve patient outcomes.
• Collaborate with community stakeholders to develop and implement health education programs that empower residents with knowledge about their health.
• Advocate for patient rights and equitable access to healthcare services, ensuring that all individuals feel heard and supported in their healthcare journeys.

Qualifications

• Current certification as a Nurse Practitioner is required.
• Active state nursing license to practice within New York.
• A Master's degree in nursing from an accredited nursing program.

Compensation and Benefits
