Work Environment

You will work onsite in a collaborative and engaging environment, serving a community of residents primarily consisting of seniors who require assistance with daily activities and access to healthcare services. The facility is designed to promote comfortable living, featuring private and semi-private rooms, communal areas for socialization, and spaces dedicated to healthcare delivery. The average workweek is approximately 40 hours, typically ranging from 9:00 a.m. to 6:00 p.m., with some evenings until 8:00 p.m. and occasional weekend outreach activities.

Key Responsibilities

• Conduct thorough health assessments, including physical exams, health histories, and screenings, to evaluate residents’ health status.
• Diagnose and manage both acute and chronic medical conditions, delivering preventive care and health promotion tailored to individual needs.
• Collaborate with staff and community organizations to create and facilitate health education programs aimed at improving community awareness and resource accessibility.
• Advocate for patients' rights, ensuring they have equitable access to healthcare services, and provide emotional support during difficult health transitions.
• Coordinate referrals and follow-up care, leveraging local health resources to guarantee continuity of care for our residents.
• Maintain and update electronic health records (EHR) accurately in compliance with healthcare regulations and best practices.
• Engage in community outreach events, health fairs, and workshops, expanding health knowledge within the community.
• Stay abreast of current trends in community health and explore innovative approaches to enhance the care provided at our facility.

Patient Population / Client Focus

Your primary focus will be on an elderly population requiring assistance with daily living activities and management of chronic health conditions. You will serve as both a clinician and an advocate, working closely with each resident to tailor interventions that respect their preferences and health goals. Your ability to connect with this population will enhance their quality of life and health outcomes.

Qualifications

• A Master’s or Doctoral degree in Nursing from an accredited program.
• Certification as a Family Nurse Practitioner (FNP), Adult-Gerontology Nurse Practitioner (ANP), or Adult-Gerontology Primary Care Nurse Practitioner (AGNP) by recognized credentialing bodies such as the American Nurses Credentialing Center (ANCC) or the American Academy of Nurse Practitioners (AANP).
• Current, unrestricted Nurse Practitioner (APRN) license in New York State.
• Possession of a valid DEA# and NPI# is required.

Preferred Experience

• Minimum of two years of clinical experience, with a preference for those who have worked in primary care or family medicine settings.
• Demonstrated strong clinical assessment and diagnostic skills with a proven track record of delivering high-quality patient care.
• Excellent communication and interpersonal skills, vital for effectively interacting with diverse populations.

Compensation and Benefits

The competitive salary for this role ranges from USD 150,000 to 160,000 annually, reflecting your skills and experience.
