Work Environment

The ideal candidate will work onsite at our Assisted Living Facility, ensuring close interactions with patients and interdisciplinary team members. The facility is equipped with state-of-the-art medical technology, facilitating effective diagnosis and management of pain and cardiovascular diseases. You will primarily follow a Monday to Friday schedule, with a total of 40 hours per week and occasional Saturday coverage.

Key Responsibilities

As a Nurse Practitioner at Wide Stream Pain Solutions, your responsibilities will include:

• Conducting thorough evaluations of patients' physical and mental health to determine their needs and establish effective treatment plans.
• Identifying and documenting both normal and abnormal findings during patient assessments.
• Developing and implementing individualized treatment plans, regularly reassessing their effectiveness and making adjustments as necessary.
• Monitoring patients' physical and emotional responses to medications and treatments, ensuring optimal management of their conditions.
• Prescribing immunizations in accordance with recommended schedules to promote preventive care.
• Collaborating closely with physicians and other healthcare providers, maintaining open communication to ensure seamless and coordinated patient care.
• Maintaining a comprehensive Problem/Medication list for each patient encounter to enhance continuity of care.
• Ensuring meticulous documentation that aligns with compliance standards for accuracy and timeliness.

Patient Population / Client Focus

Your primary focus will be on residents of the Assisted Living Facility, many of whom are managing chronic conditions that impact their quality of life. You will address both their psychological and physical health needs, aiming to enhance their daily functioning through tailored pain management strategies and holistic care approaches.

Qualifications

To be considered for this role, you must possess:

• A Master’s degree in Nurse Practitioner studies from an accredited program.
• A current Registered Nurse (RN) license in the state of New York.
• Basic Life Support (BLS) Certification.
• Strong interpersonal and communication skills that enable you to effectively interact with diverse patient populations.

Preferred Experience

While 2-3 years of experience in Cardiology is preferred, candidates with relevant experience in pain management or geriatrics will also be highly considered. This experience will enable you to bring additional insights to patient care, particularly in managing complex health issues.

Compensation and Benefits
