Key Responsibilities\n

• Conduct comprehensive ongoing assessments to accurately identify and meet the diverse needs of each patient.
\n
• Facilitate effective communication among supervisors, family members, and collateral contacts to assess and discuss the individual's progress regularly.
\n
• Provide supervisors with a planned schedule of appointments and home visits for ongoing patient monitoring.
\n
• Organize and coordinate home visits as necessary for in-home observations, follow-up interviews, and data analysis.
\n
• Ensure the effective implementation of treatment plans and regularly evaluate the appropriateness and efficacy of therapeutic interventions.
\n
• Actively participate in Interdisciplinary Team Meetings to collaboratively develop and review care strategies.
\n
• Complete thorough monthly and quarterly progress notes, ensuring documentation meets compliance standards and is readily available for review.
\n
• Provide training and support to other team members to ensure compliance with established programs and documentation processes.
\n
• Engage in regular follow-up sessions with clients to assess ongoing needs and adjustments in their therapeutic trajectory.
\n
• Maintain organized and accurate records that facilitate seamless information sharing with relevant stakeholders.
\n
• Serve as a liaison with the public and the mental health community to foster relationships that support broader developmental goals.

• Assist in designing training materials and improving program procedures to enhance service delivery for individuals.

As a Licensed Professional Counselor, you will work with a diverse clientele that includes both adults and youth facing various mental health challenges.
