Work Environment

This position offers an onsite work environment primarily situated within a school facility, allowing practitioners to interact with children and families in a familiar setting. The work schedule typically runs from 9:00 a.m. to 6:00 p.m., with some extended hours until 8:00 p.m. for evening clinics. Additionally, engagement in community outreach initiatives, including participation in local health fairs on weekends, is part of the role. The average workweek consists of approximately 40 hours.

Key Responsibilities

• Conduct thorough health assessments of students and their families to identify both immediate and long-term healthcare needs.
• Diagnose and manage a wide array of medical conditions, providing preventive care tailored to the patient's specific circumstances.
• Implement health education and promotion initiatives within the school to encourage healthy lifestyles among students and their families.
• Work collaboratively with community partners, school staff, and other stakeholders to develop comprehensive health programs that foster wellness.
• Advocate for patients, ensuring their rights are upheld while facilitating access to necessary healthcare services.
• Maintain meticulous medical records in compliance with state and federal regulations to support quality care delivery and improve health outcomes.
• Participate in community health promotion events and health fairs as part of our commitment to public health engagement.
• Stay informed on local health trends and evidence-based practices to continuously enhance the quality of care delivered within the community.

Patient Population / Client Focus

Our primary focus encompasses the health needs of school-aged children and their families, recognizing the unique healthcare challenges they face. The Family Nurse Practitioner will play a critical role in promoting health education, conducting preventive screenings, and fostering relationships that encourage a proactive approach to health within the community.

Qualifications

• Possess a current Nurse Practitioner certification.
• Hold an active nursing license in the state of New York.
• Earn a Master's degree in Nursing from an accredited institution, focusing on Family or Community Health.

Preferred Experience

• A minimum of 3-5 years of clinical experience, particularly in community health or public health settings, is strongly preferred.
• Demonstrated ability to engage effectively with diverse populations is essential.

Compensation and Benefits
