Key Responsibilities\n

• Begin discharge planning on the day of admission for all patients, ensuring a smooth transition into follow-up care.

• Conduct thorough discharge sessions with patients, emphasizing the understanding of coping skills, transition plans, and available resources.

• Confirm discharge appointments with patients and their support networks, reinforcing the continuity of care.
\n
• Maintain clear communication with referral sources and outpatient providers throughout the continuum of care: at admission, during treatment, and upon discharge.
\n
• Facilitate necessary patient transfers to higher levels of care when required, ensuring appropriate interventions are in place.
\n
• Actively participate in clinical outreach projects and program tours to enhance community engagement and promote our services.
\n
• Effectively manage crisis situations in therapy sessions and within the therapeutic milieu, providing prompt and effective interventions.

• Support milieu staff during patient interactions, enhancing their understanding of best practices for patient engagement.
\n
• Collaborate closely with the treatment team and milieu staff to uphold the highest standards of clinical quality.
\n
• Conduct assessments to evaluate patient suicidality and risks of harm to self or others, developing tailored safety plans as appropriate.

• Provide ancillary clinical support for team members in their absence, ensuring uninterrupted care for all patients.
\n
• Collaborate with the Utilization Review and Business Departments regarding authorization processes and prepare necessary documentation for payment.
