POSITION TITLE: Case Manager

REPORTS TO: CEO/Executive Director

LOCATION: Kingdom Builders Transitional Housing Program (KBTHP)

EMPLOYMENT TYPE: Full Time

Pay: $25.00 - $30.00 per hour

Position Summary:

Kingdom Builders Transitional Housing Program is seeking experienced candidates for the position of Case Manager. We are seeking dedicated individuals with strong customer service skills and a commitment to providing excellent care for our clients. Primary Responsibilities

• Assess client needs and develop, monitor, and evaluate case plans and progress.
• Facilitate interdisciplinary approaches to ensure comprehensive care for clients.
• Conduct intake interviews and review records and applications of new clients.
• Determine the need for therapeutic medical, psycho-social, and psychiatric evaluations.
• Establish treatment programs, including setting schedules and coordinating services and resources.
• Monitor clients' attendance and evaluate treatments and responses.
• Advocate for needed services and entitlements and obtain additional resources.
• Intervene in crises and provide personal support to clients.
• Maintain accurate and up-to-date client records.
• Conduct weekly interdisciplinary meetings and evaluations to communicate clients' progress.
• Provide educational resources to staff to improve their competence.
• Study and evaluate processes to improve treatment results and implement necessary changes.
• Monitor expenses and implement cost-saving actions to meet budget requirements.
• Prepare reports on case plans and results data and trends.
• Participate in educational opportunities and maintain professional networks.
• Accept ownership of new and different requests to enhance department and organization reputation.
• Connect with children effectively.

Required Qualifications:

• Strong documentation skills.
• Ability to analyze information and make informed decisions.
• Research skills to gather relevant data.
• Excellent verbal and written communication skills.
• Strong interpersonal skills to resolve conflicts and manage people effectively.
• Demonstrated integrity and commitment to providing quality care.
• Experience or knowledge in supporting individuals with mental illnesses, challenging behaviors, or formerly incarcerated individuals preferred.
• High School Diploma or GED required.
• 3+ years of customer service experience, preferably in human services.
• Must have a valid in-state driver’s license, current registration, and insurance with a clean driving record within the last three years.
• Must be over 21 years old.
• Must pass extensive background check
• Must be a certified substance use counselor

Preferred Qualifications:

• Case management: 3 years (Required)
