This Home Health Physical Therapist travel position is based in the welcoming community of Marlborough, Massachusetts, a lively city in the heart of the state’s MetroWest region. Marlborough offers an ideal blend of suburban comfort and urban convenience, making it an excellent base for a travel assignment. The city provides easy access to both Boston and Worcester, allowing you to enjoy big-city attractions while returning to a relaxed, community-oriented environment at the end of the day. Local parks, walking paths, and nearby lakes offer plenty of ways to enjoy the outdoors between shifts. For travelers, this means a rich variety of things to do, whether you prefer exploring local breweries and restaurants, attending community events, or taking day trips across the region. In this assignment, you will provide skilled home health physical therapy services to adult and geriatric patients within their homes throughout Marlborough and nearby communities. You will conduct comprehensive evaluations, develop individualized plans of care, and deliver therapeutic interventions focused on improving mobility, strength, balance, and safety in the home environment. You will also play a key role in patient and caregiver education, emphasizing fall prevention, safe transfer techniques, home exercise programs, and strategies to promote independence. A typical day involves traveling between patient homes, managing a case load that may include post-acute patients discharged from hospitals or rehab facilities, individuals with chronic conditions, and patients requiring rehabilitation after orthopedic or neurological events. You will collaborate closely with an interdisciplinary team that may include nursing, occupational therapy, speech therapy, and social work to coordinate care and support optimal patient outcomes. Documentation is completed electronically, and you will be expected to maintain timely and accurate records in alignment with home health regulations and agency protocols. The work environment is designed to be supportive and collaborative, even though the role is largely autonomous in the field. Clinical leadership and office-based staff are available to assist with complex cases, scheduling support, and clinical questions.
