This role requires experience in nursing, with an emphasis on community health education/experience. As a professional nurse, you will utilize the resources of the community to plan and direct services that meet the needs of individuals and families within their homes and communities.Duties & Responsibilities:Develop and initiate the plan of care in partnership with the patient, representative, and caregiver(s)Provide preventative and rehabilitative nursing procedures as ordered by the physicianOffer patient, caregiver, and family counseling and educationPrepare clinical notes and communicate with the physician and other healthcare practitionersIdentify discharge planning needs and implement prior to patient dischargeCommunicate with the primary physician and community health related persons to coordinate the care planParticipate in quality assessment and performance improvement programsSupervise home health aides and evaluate their careJob Requirements:Graduated from an approved school of nursing and licensed in the state of practiceMinimum of two years of experience, with preference given to public health or home care nursingCurrent CPR certificationValid driver's license and insured automobile in good working orderSelf-directed and able to work with minimal supervisionExcellent observation, problem-solving, and communication skillsAbility to organize and prioritize workload independentlyProlonged walking or standing, lifting, and physical agilityIf you are a dedicated RN looking to join a fun and loving team, apply now!
