PATH Care Navigator: Housing Stability & Support

Second Chance Center, Inc. is seeking a PATH Care Manager to provide resident-centered case management at Providence at the Heights in Aurora, CO. You will develop individualized goal plans to promote housing stability, wellness, and positive health outcomes within a Permanent Supportive Housing model. You will complete needs-based assessments, coordinate services, connect residents to income resources and benefits, and support crisis response, conflict resolution, and service navigation. #J-18808-Ljbffr Second Chance Center, Inc.
