Key Responsibilities\n

• Conduct comprehensive biopsychosocial assessments tailored to the individual needs of clients, addressing mental health, substance abuse, and medical concerns within a primary care framework.

• Deliver therapeutic services through individual and group counseling sessions, maintaining accurate electronic documentation of progress notes, treatment plans, and crisis intervention outcomes.

• Perform suicide risk assessments, escalating patient needs to appropriate healthcare providers as required, ensuring that safety and well-being remain top priorities.

• Serve as a liaison between clients and local resources, facilitating access to community services and support systems, including medication assistance and rehabilitation programs.

• Collaborate with Quality Improvement teams to develop and refine processes for data collection, ensuring that our services remain evidence-informed and outcome-oriented.

Qualifications\n

Compensation and Benefits\n
