Key Responsibilities\n

• Initiate detailed discharge planning on the day of patient admission, collaborating closely with clinical teams to prepare tailored transition plans.

• Conduct comprehensive discharge sessions to help patients understand their coping strategies, transition plans, and available resources, ensuring clarity and confidence in their post-discharge plans.
\n
• Verify and confirm discharge appointments with patients and their families, promoting engagement and preparedness for the next steps in their care journey.
\n
• Maintain open lines of communication with referral sources and outpatient providers to guarantee continuity of care at all critical points: admission, midpoint, and discharge.
\n
• Facilitate patient transfers to higher levels of care when required, diligently assessing each situation to ensure optimal patient safety and support.
\n
• Actively engage in clinical outreach activities and participate in program tours to represent the organization and its services.
\n
• Manage crises effectively within therapeutic sessions and the surrounding milieu to ensure safety and stability for all clients.
\n
• Support milieu staff in their interactions with patients, providing guidance and enhancing the quality of client relationships.
\n
• Collaborate with the treatment team to maintain a high standard of clinical quality across all areas of care.
\n
• Complete necessary documentation, including initial assessments, treatment plans, and case notes in a timely manner, ensuring compliance with all regulatory standards.

• Assess patient risks related to suicidality and safety, providing critical support and interventions as necessary.
\n
• Communicate regularly with the Utilization Review Department and Business Department regarding authorizations and billing, ensuring that services rendered are well documented and reimbursed appropriately.

The LCSW will work with a diverse clientele, including individuals facing a variety of mental health challenges such as depression, anxiety, trauma-related disorders, and other behavioral health concerns. Qualifications\n

• A Master’s degree in Social Work, Counseling, Psychology, or a related mental health discipline is required.

• Familiarity with Office 365 and Electronic Health Record systems to efficiently document patient interactions and care plans.

• Demonstrated experience in completing documentation, including initial assessments and SMART treatment plans.

• Excellent communication skills, both written and verbal, to effectively engage with clients, team members, and external partners.

Preferred Experience\n

While not mandatory, preferred candidates will have previous experience working within community health settings and demonstrate an understanding of the unique challenges faced by diverse populations. Experience with crisis intervention and mental health advocacy is highly valued.
