The Enlightened Mind Counseling Center is seeking a Licensed Master Social Worker (LMSW) to provide compassionate and comprehensive support for seniors in our community health clinic. This position is fully remote, offering flexibility in your work schedule while adhering to essential hours for client engagement. Typical workdays are Monday through Friday, from 8:00 AM to 4:30 PM, with opportunities for evening or weekend hours based on client needs. As a Social Worker in this role, you will utilize telehealth platforms to conduct assessments and follow-ups, ensuring you can meet seniors where they are most comfortable. Occasional on-call duties may be required to provide immediate support during urgent situations.

You will be tasked with a variety of responsibilities designed to ensure the well-being of our senior clients:

• Conduct in-depth psychosocial assessments to determine clients' emotional, mental, and social needs, using validated tools and frameworks.

• Develop individualized care plans in collaboration with seniors and their families, setting goals that enhance their overall quality of life.

• Offer ongoing counseling sessions, providing emotional support as clients navigate life transitions, grief, or health challenges.
\n
• Advocate for clients by connecting them to community resources, ensuring they have access to necessary services such as healthcare, financial aid, and support groups.
\n
• Engage with interdisciplinary teams, including nurses, healthcare providers, and caregivers, to coordinate comprehensive care and address any barriers to service delivery.

• Implement regular check-ins through telehealth visits to monitor the psychological well-being of seniors, adjusting care as needed.
\n
• Facilitate discussions among family members to address conflicts and ensure that each member understands the care strategy.
\n
• Provide educational resources to clients and their families regarding social services, community programs, and support networks available to enhance social engagement.
\n
• Maintain meticulous records of assessments, care plans, and progress notes, ensuring compliance with legal and ethical standards.

• Stay updated on best practices and policies within social work, actively participating in continuing education opportunities.

Your primary focus will be on the senior population, particularly those facing challenges related to aging, including cognitive decline, physical disabilities, and social isolation. By understanding their unique needs, you will create a supportive environment that respects their dignity and autonomy while promoting their health and well-being.

• Master's degree in Social Work from an accredited institution.
\n
• Current and valid LMSW licensure granted by the state of Pennsylvania.
\n
• At least two years of experience in social work, with a preference for roles focused on elderly care or within community health settings.

• Strong understanding of the issues impacting seniors, including mental health, social support systems, and healthcare access.
\n
• Exceptional problem-solving, assessment, and active listening skills, with the capacity to empathize and communicate effectively with a diverse clientele.

• Adept in electronic health records (EHR) systems and online communication tools to facilitate remote service delivery.
\n

While a minimum of two years in social work is required, experience working in geriatric care settings, community health initiatives, or similar environments is highly desirable. Familiarity with community resources in Philadelphia and an ability to navigate bureaucratic processes to assist clients effectively will be an asset.

The salary for this position ranges from $60,000 to $100,000 annually, commensurate with experience and qualifications. In addition to competitive compensation, our benefits package includes:
