Work Environment\n

This position is remote, allowing for flexibility in your work schedule while still enabling you to make meaningful home visits with clients and families. Although the primary hours are Monday through Friday from 8:00 AM to 4:30 PM, candidates should be willing to accommodate occasional evenings and weekends depending on client needs. This flexibility is essential as we strive to provide consistent support. Furthermore, being on-call may be necessary at times to respond to urgent client situations.

Key Responsibilities\n

• Conduct in-depth assessments of clients' psychosocial needs and formulate tailored care plans that align with their preferences and goals.

• Provide direct counseling and emotional support, guiding clients and their families through significant transitions and complex decisions related to end-of-life care.

• Act as an advocate for clients to ensure their access to critical community resources, facilitating access to services such as transportation, meals, and support groups.

• Collaborate with an interdisciplinary team of nurses, caregivers, and healthcare professionals to coordinate care and optimize client well-being.

• Regularly visit clients to monitor their mental and emotional states, assess living conditions, and proactively address any emerging safety concerns.

• Facilitate communication regarding care among family members, resolving conflicts to enhance the support system around each client.

• Educate clients and families on available community resources, including emotional support services and financial assistance programs.

• Maintain meticulous documentation of assessments, care plans, and interactions using electronic health record systems to ensure compliance with healthcare regulations.

Qualifications\n

Compensation and Benefits\n

We offer a competitive salary ranging from $60,000 to $100,000 per year, commensurate with experience.
