This role is structured within a remote setting, allowing you to connect with clients from the comfort of their homes and various community locations. While the position is primarily remote, occasional in-person meetings may be required at our Community Health Clinic in Philadelphia to collaborate with other healthcare professionals. Key Responsibilities\n

• Conduct regular assessments and evaluations of seniors’ emotional, social, and mental health needs through in-depth interviews and observations.

• Develop personalized care plans in concert with clients and their family members, tailoring each plan to address individual goals and preferences.

• Provide one-on-one counseling sessions to seniors, addressing life transitions such as loss, health changes, and family dynamics.

• Advocate passionately for clients’ rights, ensuring access to necessary services, community resources, and support systems, including healthcare, financial aid, and social groups.

• Engage with interdisciplinary teams, including nurses, caregivers, and social workers, to coordinate comprehensive care and enhance overall treatment outcomes.

• Conduct home visits and community outreach, evaluating clients’ living conditions and promptly addressing any safety or health concerns.

• Assist in family conflict resolution concerning the care and well-being of senior clients, educating family members on effective caregiving strategies.

• Stay informed of current social work policies and best practices to ensure compliance with ethical standards in all interactions and documentation.

• Utilize electronic health record (EHR) systems to maintain accurate client files, documenting assessments, care plans, communications, and progress notes efficiently.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution.

• Current and valid licensure as a Licensed Master Social Worker (LMSW) in Pennsylvania.

Compensation and Benefits\n
