Key Responsibilities\n

• Perform comprehensive assessments of seniors’ psychosocial needs, accounting for emotional, mental, and social dimensions.

• Develop and implement individualized care plans in collaboration with clients and their families, ensuring these plans align with their expressed preferences and objectives.

• Provide counseling and emotional support, facilitating greater resilience in clients as they navigate life transitions, such as moving to assisted living or coping with loss.

• Act as an advocate for clients, ensuring they have access to essential community resources and support services.

• Coordinate with a multidisciplinary team—including nurses, healthcare professionals, and community organizations—to ensure cohesive and comprehensive client care.

• Conduct regular check-ins with clients to monitor progress and address any safety or living condition concerns, adjusting care plans as necessary.

• Foster communication between family members, mediating any conflicts and promoting a supportive family environment.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution is required.
