Key Responsibilities\n

• Conduct in-depth psychosocial assessments to understand the emotional, social, and environmental factors affecting patients and their families.

• Work in collaboration with the medical team to develop and implement individualized care plans that address each patient's unique needs.

• Provide individual and group therapy sessions, focusing on grief counseling, coping strategies, and emotional well-being.

• Facilitate communication among family members, caregivers, and healthcare providers to explain treatment options and available resources.

• Assist patients and families in navigating post-discharge resources, ensuring continuity of care and support beyond the hospice setting.

• Deliver crisis intervention services as needed, providing support during challenging emotional or medical situations.

• Maintain accurate and timely documentation in accordance with hospital policies and legal standards, ensuring compliance with all regulations.

Patient Population / Client Focus\n

The primary focus of this role will be to support hospice patients, many of whom face life-limiting illnesses. You will provide compassionate care and emotional support to individuals across various backgrounds, ensuring they and their families feel heard, valued, and understood. Your work will include addressing behavioral health issues related to grief, anxiety, depression, and adjustment to terminal diagnoses, ensuring they have the resources and support necessary during their healthcare journey.

• A Master’s degree in Social Work from an accredited institution.

• Active state licensure as a Licensed Clinical Social Worker (LCSW).
