Key Responsibilities\n

• Serve as an essential liaison between families, outpatient providers, and interdisciplinary teams to ensure cohesive care delivery.

• Engage with insurance companies to facilitate pre-certification and manage utilization effectively, streamlining access to necessary services.

• Participate in comprehensive discharge planning and aftercare strategies, utilizing evidence-based approaches to optimize patient transitions and support continuity of care.

• Provide therapeutic meal support and education, helping clients develop a healthy relationship with food through modeled behaviors and therapeutic techniques.

• Deliver safe, empathetic, and affirming care tailored to each client’s unique circumstances, considering their intersecting identities and backgrounds.

Patient Population / Client Focus\n

This position primarily focuses on working with older adults receiving hospice care, who may be navigating complex emotional and psychological challenges associated with end-of-life issues. Qualifications\n

• A Master's degree in Social Work from an accredited institution, ensuring a solid foundation in clinical practices.

Compensation and Benefits\n
