This full-time position offers a competitive annual salary ranging from USD 70,000 to 100,000, commensurate with experience and education. You will be collaborating closely with a multi-disciplinary team, including the Clinical Director and Executive Director, ensuring that therapeutic environments and routines are maintained effectively for our patients. Key Responsibilities\n

As a Licensed Clinical Social Worker, your key responsibilities will include:

• Conducting thorough assessments of clients to determine their counseling needs and establishing appropriate treatment plans.

• Evaluating clients for substance abuse diagnoses using the DSM (Diagnostic and Statistical Manual) to recommend suitable levels of care in accordance with ASAM (American Society of Addiction Medicine) standards.

• Developing and documenting substance abuse counseling components of client treatment plans in collaboration with the treatment team.
\n
• Creating comprehensive discharge and continuum of care plans that facilitate each client's progress through various levels of support.
\n
• Maintaining meticulous clinical records, ensuring that all counseling sessions, assessments, referrals, and follow-up activities are documented accurately.

• Managing a caseload of clients, performing case management duties, and participating in utilization review updates as required.
\n
• Coordinating support services for clients and their families, keeping detailed records of services provided.
\n
• Contacting and working with external referral sources, including legal, educational, and occupational contacts, to integrate care effectively.

• Utilizing a multi-disciplinary team approach to facilitate interventions for clients dealing with crisis situations, ensuring they receive timely and effective support.

Patient Population / Client Focus\n

Your work will primarily focus on patients in a hospice setting, many of whom may be managing dual diagnoses involving substance use and mental health disorders. This role requires sensitivity to the unique needs of clients in crisis, understanding that they may be navigating complex life situations. By providing direct counseling services—including individual, family, and group therapy—you will enhance the therapeutic experience, fostering an environment conducive to long-term recovery and healing.

Qualifications\n

To be successful in this role, candidates must possess the following qualifications:

• A Bachelor’s Degree in Social Work or a related field, with a valid LCSW license in Illinois.
