This position is fully remote, allowing you to work comfortably from your home in Denver, Colorado, or anywhere else in the state. You will collaborate with a diverse team of mental health professionals while providing care within a hospice center environment. Here, you will engage with patients who require sensitive and specialized support, ensuring a personalized approach to their mental and emotional needs.

Key Responsibilities\n

• Conduct comprehensive psychosocial evaluations to create accurate diagnoses and treatment plans for patients experiencing acute and chronic mental health issues.

• Design and implement tailored therapeutic strategies that cater to the unique needs and preferences of each client, employing various evidence-based practices.

• Deliver therapeutic interventions through modalities such as solution-focused therapy, behavioral activation, problem-solving treatment, and motivational interviewing. For instance, you may conduct weekly therapy sessions focusing on grief counseling for clients in hospice care.

• Regularly monitor and document patients' progress toward treatment objectives, ensuring that adjustments are made as necessary, whether via video calls or in person.

• Collaborate with an interdisciplinary team, including physicians, psychologists, and other health professionals, to ensure coordinated and comprehensive care. Share insights during team meetings to enhance integrated patient care.

• Support the safe administration and monitoring of psychotropic medications as outlined by medical providers, focusing on treatment adherence and any observed side effects. This involves close communication with both providers and patients to optimize mental health outcomes.

Patient Population / Client Focus\n

The primary focus for this role involves working with clients within hospice care, who often face complex emotional and psychological challenges, including grief, anxiety, and depression. Your expertise will play a crucial role in helping these individuals and their families navigate through difficult transitions, fostering emotional resilience and promoting overall mental wellness.

Qualifications\n

• A Master's degree in Social Work from an accredited institution.

• Current, valid, and unrestricted LCSW license to practice independently in the state of Colorado.

• Minimum of 1 year of relevant experience as an independently licensed therapist, ideally within a mental health or hospice care setting.

Compensation and Benefits\n
