Key Responsibilities\n

• Conduct thorough assessments that identify a client’s social, emotional, and environmental needs, ensuring a complete understanding of their circumstances.

• Develop and implement individualized service plans that encompass treatment goals, interventions, and methods for progress tracking.

• Provide direct counseling and emotional support to individuals and families facing end-of-life challenges, helping them navigate grief, loss, and other related issues.

• Collaborate with clients to create tailored service plans that connect them to necessary community resources, promoting self-sufficiency and empowerment.

• Deliver crisis intervention services when necessary, incorporating risk assessments and safety plans to ensure client wellbeing.

• Work in partnership with community agencies and organizations to coordinate services and advocate for the interests of clients.

• Participate in community outreach efforts aimed at raising awareness of hospice services and enhancing access to support.

• Engage in regular case conferences, attend team meetings, and partake in staff training and development opportunities.

Patient Population / Client Focus\n

In this role, you will be primarily focused on serving individuals and families dealing with terminal illnesses, ensuring that they receive not only medical assistance but also emotional and psychological support. Qualifications\n

Compensation and Benefits\n
