Key Responsibilities\n

• Conduct comprehensive psychosocial assessments to evaluate patients' emotional, psychological, and social needs, integrating this information into individualized care plans.

• Provide individual and family therapy sessions, focusing on emotional support for both patients and their loved ones, utilizing evidence-based therapeutic approaches such as Cognitive Behavioral Therapy and Solution-Focused Brief Therapy.

• Implement and oversee crisis intervention strategies, offering risk assessment and safety planning aimed at fostering a safe environment for patients and families.

• Work closely with a multidisciplinary team of medical professionals, including physicians and nurses, to ensure seamless and holistic patient care.

• Maintain accurate and timely documentation of patient interactions and treatment progress, adhering to ethical and legal standards to uphold trust and transparency.

• Facilitate group therapy sessions and psychoeducational workshops to provide community support and mental health education.

Patient Population / Client Focus\n

In this role, you will primarily work with patients who are at the end stages of life, facing terminal illnesses and the associated emotional distress. Qualifications\n

• A Master’s degree in Social Work from an accredited program.

• Current and active state licensure as a Licensed Independent Clinical Social Worker (LICSW).

Compensation and Benefits\n
