This remote position will primarily serve clients within our Hospice Center in Tampa, Florida. Key Responsibilities\n

• Conduct comprehensive psychosocial assessments through patient interviews, detailed observations, and thorough reviews of medical records to understand the emotional, social, and environmental factors affecting recovery.

• Provide one-on-one counseling and emotional support to both patients and their families, helping them navigate adjustment issues, coping mechanisms, and emotional well-being during the rehabilitation process.

• Work collaboratively with the rehabilitation team to create and implement individualized treatment plans that address the unique psychosocial needs of each patient.

• Facilitate access to community resources for support groups, vocational rehabilitation, housing assistance, or other essential services that may aid in the recovery and rehabilitation of patients.

• Educate patients and families about available community resources and post-rehabilitation options to ensure a smooth transition to home or other care settings.

• Participate actively in discharge planning meetings to ensure comprehensive care throughout the patient's rehabilitation journey.

Compensation and Benefits\n
