Key Responsibilities\n

• Conduct comprehensive psychosocial assessments to collect detailed information regarding clients’ mental health concerns, strengths, and available resources, ensuring a thorough understanding of the client's situation.

• Provide individual, family, and group therapy by employing evidence-based treatment modalities such as cognitive-behavioral therapy, solution-focused therapy, and trauma-focused therapy tailored to the unique needs of clients experiencing grief and loss.

• Develop and implement individualized treatment plans that include specific goals and therapeutic interventions aligned with the client’s and family's evolving needs throughout the hospice process.

• Coordinate with a multidisciplinary team by collaborating with medical professionals, including psychiatrists and psychologists, to establish comprehensive, client-centered care plans that encompass all facets of the patient's care.

• Respond to crisis situations by providing immediate crisis intervention, conducting risk assessments, and planning safety interventions to support clients in distress.

• Facilitate group therapy sessions, psychoeducation workshops, and support groups focused on grief, loss, and coping strategies for families dealing with terminal illness.

• Ensure meticulous and timely documentation in compliance with ethical and legal standards to maintain high-quality records of treatment and progress.
