Key Responsibilities\n

• Conduct thorough psychosocial assessments to gather in-depth information regarding clients’ mental health concerns, including their strengths and available resources.

• Provide direct therapeutic interventions through individual, family, and group therapy sessions, utilizing evidence-based treatment modalities tailored to client needs.

• Develop and implement personalized treatment plans that highlight specific goals and interventions, regularly evaluating progress and making necessary adjustments.

• Engage collaboratively with a multidisciplinary team, including psychiatrists and psychologists, to ensure coordinated and effective treatment strategies for all clients.

• Offer crisis intervention and risk assessment services, employing best practices to ensure client safety and immediate support when required.

• Maintain accurate and timely documentation of client interactions in compliance with ethical and legal standards, ensuring confidentiality and professionalism at all times.

• Facilitate psychoeducation classes, support groups, and therapy sessions, encouraging group interaction and shared learning among clients.

Patient Population / Client Focus\n

In this role, you will primarily serve clients facing terminal illnesses and their families, providing emotional support and counseling through what can be one of the most challenging times in their lives. Sensitivity, empathy, and an understanding of grief and loss are critical to effectively support this unique patient population.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution.

• An active state licensure as a Licensed Clinical Social Worker (LCSW).

• A minimum of two years’ experience in a mental health or outpatient setting, especially within hospice or palliative care environments.

• Strong familiarity with evidence-based therapeutic modalities and treatment approaches.

Compensation and Benefits\n
