Key Responsibilities\n

• Conduct comprehensive assessments of clients’ mental health needs through telehealth sessions, ensuring each client's unique requirements are addressed.

• Collaborate with a multi-disciplinary healthcare team including supervisors, direct care staff, and family members to monitor progress and refine treatment plans.
\n
• Maintain consistent communication regarding appointments and progress notes, providing supervisors with scheduled updates on all client interactions.
\n
• Coordinate and document home visits (when required) to observe clients' environments and behaviors, analyzing data to support individualized care plans.
\n
• Effectively implement and evaluate program plans, ensuring that each strategy is tailored to achieve optimal outcomes for clients.
\n
• Participate actively in Interdisciplinary Team Meetings to share insights, discuss challenges, and collaboratively improve client care strategies.
\n
• Complete detailed monthly and quarterly progress notes demonstrating clients' advancements toward their treatment goals and adherence to strategies.
\n
• Organize training sessions for both staff and team members to ensure competent implementation of treatment plans and efficient documentation practices.
\n
• Provide ongoing follow-up through home visits, interviews, and observations to continuously support client needs and foster positive outcomes.
\n
• Maintain accurate records to facilitate information sharing with stakeholders while ensuring compliance with legal and ethical standards within mental health practices.

• Act as a liaison with the mental health community and public organizations to drive collaborative development goals that enhance service delivery.

• Assist in developing training procedures and materials that promote excellence in program delivery and professionalism among staff members.

As an LPC at our center, you will be working with a diverse population, including adults, youth, and families facing various mental health challenges. Qualifications\n

• A Master’s or Doctoral degree in Counseling, Psychology, or a related field is required.

• Active and unrestricted Professional Counselor (LPC) license in the state of Texas.

• A minimum of two years of clinical counseling experience, demonstrating competence in providing comprehensive mental health care.

• In-depth knowledge of ethical and legal standards in the counseling profession.
