You will engage with clients through various telehealth platforms, ensuring that you can effectively connect with them from the comfort of their homes while prioritizing their privacy and safety. Key Responsibilities\n

• Utilize available community resources on behalf of patients and families, ensuring they have the support they need.

• Educate patients and their families about discharge options based on mental health, legal, and insurance considerations, to facilitate a smooth transition.
\n
• Provide continuous support and referrals for ongoing community services and resources even after discharge, to promote sustained recovery.
\n
• Coordinate and schedule follow-up appointments with psychiatric providers within a week of discharge to ensure ongoing care.
\n
• Execute thorough safety planning in collaboration with patients and families, including measures such as firearm removal when necessary for safety.
\n
• Communicate effectively with interdisciplinary healthcare teams, legal representatives, and patient advocates to expedite patient progress while preserving patient rights and ensuring facility safety.

Patient Population / Client Focus\n

Your primary focus will be on diverse patients grappling with various mental health challenges, from anxiety and depression to more complex behavioral issues. Conducting assessments and developing tailored therapeutic plans, you will work closely with families to address specific needs, foster understanding, and restore stability in their lives.

Qualifications\n

• A valid and active state license as a Licensed Marriage and Family Therapist (LMFT).

• A Master’s degree in Marriage and Family Therapy or a closely related field.
\n
• At least 2 years of relevant experience in the mental health field, preferably in outpatient settings.

Preferred Experience\n

• Experience with telehealth services is highly desirable, as well as familiarity with electronic health record (EHR) systems.

• Demonstrated ability to work collaboratively within interdisciplinary teams and with diverse patient populations.

• Background in crisis intervention and management can be advantageous.

Compensation and Benefits\n

This role offers a competitive salary range of USD 120,000 to 130,000 annually, commensurate with experience. Additional benefits include flexible working hours, generous leave policies, and a commitment to work-life balance.
