Key Responsibilities\n

As a Senior Licensed Clinical Social Worker, your primary responsibilities will include:

• Conducting comprehensive clinical assessments to understand patients’ mental health needs.

• Developing individualized treatment plans based on evidence-based practices and patient preferences.

• Utilizing a variety of therapeutic modalities, such as cognitive-behavioral therapy (CBT) and dialectical behavior therapy (DBT), tailored to the hospice population's unique circumstances.

• Maintaining meticulous clinical records that comply with HIPAA standards and contribute to effective care coordination.

• Collaborating with an interdisciplinary team, including psychiatrists and psychologists, to create a holistic care approach for each patient.

• Facilitating regular case consultations and peer review meetings to enhance care quality and address complex cases collaboratively.

• Providing psychoeducation and support to patients and their families regarding mental health concerns, treatment modalities, and available resources.

In this role, you will be primarily working with a hospice patient population, many of whom may be facing terminal illnesses and the emotional complexities associated with end-of-life care. You will help patients and families navigate grief, anxiety, and depression while promoting emotional resilience and enhancing overall well-being.

Qualifications\n

The ideal candidate for this role will possess:

• A valid license to practice as a Licensed Clinical Social Worker (LCSW) in the state of California.

• A Master’s or Doctorate degree in Social Work from an accredited institution.

• A minimum of 2 years of clinical experience, particularly in providing psychotherapy services.

• Proficiency in evidence-based treatment techniques, especially in solution-focused interventions.

• Strong communication and interpersonal skills, capable of establishing rapport with patients and collaborating effectively with a diverse team.

Preferred Experience\n
