Work Environment\n

This role is primarily remote, enabling flexibility while ensuring consistent access to clients across our hospice center. While standard working hours are Monday to Friday from 8:00 AM to 4:30 PM, there may be occasional evening or weekend requirements based on client needs. You may also be called upon for urgent matters, necessitating a degree of on-call availability to ensure continuous care. This setup supports your work-life balance while allowing you to contribute directly to client well-being.

Key Responsibilities\n

• Conduct comprehensive assessments of seniors’ psychosocial needs, focusing on emotional, mental, and social factors influencing their health and well-being.

• Collaborate with clients and families to create tailored care plans that align with their individual preferences and goals, promoting an enriched living experience.

• Provide counseling and emotional support to clients, assisting them in navigating complexities such as life transitions, mental health issues, or caregiving demands.

This role centers on working with senior clients primarily within a hospice setting. You will support individuals facing the challenges associated with aging, chronic illness, and end-of-life transitions. Qualifications\n

• Master's degree in Social Work from an accredited institution.

• Current LICSW licensure in Massachusetts.

• A minimum of two years of professional experience in a social work role, ideally within a geriatric or community-based setting.

Compensation and Benefits\n
