Key Responsibilities\n

  Conduct comprehensive assessments of clients’ mental health needs, utilizing the Diagnostic and Statistical Manual of Mental Disorders as necessary to formulate tailored care plans that integrate cultural competence and address individual patient needs.

  Continuously monitor patient progress through regular evaluation and adjustment of treatment strategies, ensuring that interventions effectively support mental health diagnoses and overall wellness.

  Document all treatment records, clinical reports, and other essential information accurately and timely using our electronic medical record system, in accordance with regulatory requirements.

  Collaborate with patients and their families to co-develop actionable treatment plans that reflect personal goals, address safety concerns, and promote symptom management.

  Coordinate referrals to additional services and establish connections with healthcare providers to ensure a comprehensive approach to patient care.

  Provide educational support and mentorship to students and interns within the organization, enhancing their learning and developing their skills in clinical settings.

  Participate in peer review and clinical collaboration activities, contributing to a supportive and engaging team culture aimed at enhancing performance and care quality.

Patient Population / Client Focus\n

The LCSW will work with diverse patient populations within our community health clinic, offering services primarily to individuals facing a range of mental health challenges. This includes working with adults, children, and families from various cultural and socioeconomic backgrounds who may experience anxiety, depression, trauma, and more.   A Master’s degree in Social Work from an accredited institution is required.

Preferred Experience\n

Compensation and Benefits\n

We offer a competitive salary package ranging from $90,000 to $105,000 annually, commensurate with experience.
