Key Responsibilities\n

• Work closely with medical teams to create integrated care plans that respond to the patient's unique psychosocial challenges.

• Deliver individual and group counseling sessions to support patients and their families in processing emotions associated with illness, bereavement, and other stressors.

• Coordinate with community organizations and service providers to arrange for post-discharge support services.

• Engage in crisis intervention as needed, providing immediate support and effective coping strategies for patients facing urgent situations.

• Assist in developing personalized discharge plans that incorporate community resources to ensure a smooth transition post-care.

• Maintain clear, accurate, and timely documentation in compliance with hospital policies and legal requirements for social work practice.

• Participate in multidisciplinary meetings to enhance patient care strategies and share insights related to social work practices within the hospice context.
