Key Responsibilities\n

• Engage actively with a variety of community organizations and agencies to coordinate comprehensive care for clients.

Patient Population / Client Focus\n

Your work will primarily involve supporting patients and families navigating the complexities of hospice care. Qualifications\n

• A Master’s degree in Social Work from an accredited institution is required.

Compensation and Benefits\n
