Speech-Language Pathology Inpatient & Home Care Clinical Coordinator
Cincinnati Children's Hospital Medical Center is seeking a full time (1.0 FTE) Speech-Language Pathology Clinical Coordinator. The schedule for this position requires Monday-Friday on site schedule. The job responsibilities for this position will be a split between the Speech Pathology Home Care program and main campus inpatient clinical care environment.
Primary responsibilities include clinical care, personnel administration, clinical leadership, division leadership, and professional leadership.
Home Care
- Provide some direct patient care to Home Care patients;
- Provide oversight and supervision of the Speech-Language Pathologists providing direct patient care to home care patients.
- Participate in recruitment, interviewing, hiring, orientation, onboarding of all new Home Care Speech Pathologists.
- Ensure all workflow, epic education, clinical mentoring, TJC preparedness are reinforced.Ensure completion and documentation of all Speech-Language Pathology and Home Care competencies are completed at baseline and when due for renewal;
- Ensure patients are recertified according to criteria: monitor/verify that progress reports and recommendations are completed for patients every 60 days for HC recertification
- Complete peer reviews for SLP team members; complete annual performance reviews for all direct reports;
- Update shared drive with documentation reflecting completion of orientation, current licensure, HHC Speech-Language Pathology Job Aids, and HHC SLP meeting minutes
- Plan and lead quarterly SLP Home Care meetings and file minutes of discussions.
- Agendas to include SLP education on HHC policy updates/changes; case management; case studies, general team business, EPIC education, and any quality improvement/project work related to home care.
- Regulatory knowledge of home care including expectations, competencies, safety, infection control, and clinical documentation as it pertains to The Joint Commission (TJC) and Centers for Medicare and Medicaid Services (CMS)
- Conduct biannual supervisory visits for each SLP on the team to monitor infection control, hand washing, and TJC readiness; document observations in VERGE audits; maintain infection control tracking.
- Managing assignment of patients including new agency, Starshine Speech Pathology/Hospice patients
- Oversight of SLP clinical schedule, availability, and upcoming openings to allow efficient scheduling of patients
- Consistent awareness of patient scheduling and staff productivity to allow increased scheduling, flexible scheduling to meet patient care and agency needs
- Assure assignment and continued care of patients meet home care criteria
- Additional Home Care responsibilities:
- Coordinate with Home Care to participate in setting, revision of staff education related to HHC policy and procedures
- Maintain active participation in and management of monthly multi-disciplinary case conferencing process for HHC patients
- Participate in and contribute to monthly HHC Blitz education
- Active participation in HHC professional governance
- Attend all epic upgrade training and cascade information to SLP HC team through in person training, phone or email support as appropriate
- Attend any additional discretionary meetings with HHC (including HHC Ed Specialists, Clinical Coordinators, Managers) to receive training and cascade to SLP HC team.
Main Campus Inpatient
- Serve as mentor for staff working on various clinical competencies to ensure consistent expectations with methods of practice and alignment with inpatient SLP team
- Work with inpatient SLP leadership team to identify and implement strategies to improve workflows and processes
- Provide management support for inpatient Speech-Language Pathologists
Additional Information Related To JOB RESPONSIBILITIES
- Clinical Care and Follow-up: Performs as an expert diagnostician and therapist, including as mentor/supervisor for students and all levels of clinicians. Administers appropriate tests according to standard procedures; makes accurate diagnoses, appropriate recommendations and referrals. Counsels parents and provides appropriate handouts and home programs. Uses best practice decision-making to determine the appropriate frequency and duration of therapy. Formulates appropriate goals for every patient in treatment. Involves parents in treatment plan and treatment process. Structures sessions and selects appropriate intervention strategies to accomplish goals. Documents progress daily and updates treatment plan as appropriate. Completes all required documentation and charges within the standard time frames. Determines discharge criteria and prepares patient for discharge or transition as appropriate. Manages schedule and patient visits to make productive use of time and achieve billing targets. Anticipates scheduling variability and make adjustments in schedule accordingly.
- Personnel Administration: Participates in determining clinical and specialty needs at assigned location. Assists with orientation of new staff. Provides supervision, mentoring, coaching, goal setting, development, counseling and recognition for assigned staff. Conducts performance evaluation for assigned staff and assures completion of competencies, mandatory training, continuing education and licensure requirements. Coordinates the clinical activities of the staff and manages the schedules and office/treatment room assignments. Collaborates with the Clinical Manager on staff scheduling and staff metrics. Communicates with staff through regularly scheduled staff meetings.
- Clinical Leadership: Maintains at least 2 advanced competencies and serves as a clinical specialist. Helps to ensure that processes and procedures are state‑of‑the‑art, current, effective and efficient. Promotes evidence‑based practice. Serves as role model of cooperation, respect, and trust. Promotes a positive work environment. Determines appropriate equipment and materials needed for assigned location. Is responsive to customer needs and implements solutions to identified problems. Is knowledgeable of relevant procedures and regulations and ensures compliance at assigned location. Demonstrates fiscal responsibility; identifies and implements strategies to reduce operating costs. Assists in preparation of the budget for the assigned area. Supervises ordering and stocking of supplies and materials.
- Division Leadership: Represents SLP division on site and hospital work groups/committees as appropriate. Represents SLP division and CCH in a professional organization or special interest group at state or national level. Responds to the need for work on special projects or development of new procedures. Recognizes opportunities for improvement and generates performance improvement activities. Stays current with professional issues related to clinical practice and operational management. Is an active participant on division Leadership Team.
- Professional Leadership: Stays current with advances in speech‑language pathology practice by regularly reviewing the research literature, consulting the Practice Management section of the ASHA website, including the Practice Portal, and regularly participating in continuing education to supplement advances in the profession and information in the scope of practice. Plays critical role in supporting health literacy through division materials and communications, screening and diagnosis, and prevention practices. Stays current in understanding and use of the International Classification of Functioning, Disability and Health for Children & Youth (ICF-CY), the DSM, and CPT codes to support clinical and fiscal activities. Contributes to local, regional, and national conversations involving relevant professional issues.
In addition to demonstrating advanced clinical skills and maintaining engagement in the five domains of professional practice, the Coord: Speech Lang Pathologist engages in activities to coordinate and manage a program or clinical site, including personnel, service delivery, and budgetary responsibilities.
Job Qualifications
- Master's degree in speech-language pathology or communication sciences & disorders
- Active license in speech-language pathology in the state of practice
- Hold Certificate of Clinical Competence (CCC) granted by American Speech-Language-Hearing Association (ASHA)
- Maintain core and advanced Speech-Language Pathology competencies in birth-three, dysphagia complex, and AAC
- 8+ years of work experience in a related job discipline
