• Family Consultation: Engage with clients' families and support systems to provide guidance on treatment plans and address their concerns regarding the patient’s journey.

• Care Coordination: Assist clients in accessing services post-discharge by connecting them with additional resources and providers within the community.

Qualifications\n

Compensation and Benefits\n
