Key Responsibilities\n

As an LPC at Primary Counseling Center, your day-to-day responsibilities will include:

• Conducting comprehensive assessments to accurately understand the needs and goals of each patient.
\n
• Actively communicating with supervisors, direct care staff, family members, and other stakeholders to provide updates on patient progress.
\n
• Developing tentative schedules for various appointments and home visits, ensuring effective monitoring of work progression.
\n
• Coordinating necessary home visits which include in-home observations, chart reviews, and staff interviews to gather important data for patient care.
\n
• Implementing and overseeing program plans with attention to detail, ensuring that they are carried out effectively and efficiently.
\n
• Engaging in interdisciplinary team meetings to collaborate on patient care strategies and share insights from your assessments.
\n
• Completing detailed monthly and quarterly progress notes to document patient advancements and challenges.
\n
• Leading training sessions that certify staff members in the proper implementation of program plans and documentation processes.
\n
• Providing follow-up home visits and interviews to ensure ongoing support tailored to individual needs.
\n
• Maintaining thorough and organized records to facilitate information access for all relevant parties.
\n
• Acting as a liaison within the mental health community to enhance public understanding and support for our service goals.

• Assisting in the development of training materials aimed at continually improving our programs.

Patient Population / Client Focus\n

You will work with a diverse client base that includes both adults and youth, addressing various mental health challenges across different demographics. Our center’s focus is to provide inclusive care, especially to those who may have limited access to traditional mental health services, whether due to location, economic constraints, or social stigma.

Qualifications\n

The ideal candidate will possess the following qualifications:

• A Master’s or Doctoral degree in Counseling, Psychology, or a related field.

• Active, unrestricted licensure as a Professional Counselor (LPC) in the state of Texas.
\n
• At least two years of experience in a clinical counseling setting.
\n
• Demonstrated knowledge of legal and ethical standards governing mental health practices.
\n
• Strong interpersonal and communication skills, with a proven ability to build rapport with a diverse clientele.

• Experience collaborating effectively with a multidisciplinary healthcare team.
\n
• Proficiency in electronic medical records (EMR) software and other related technology.

Compensation and Benefits\n
