Key Responsibilities

• Evaluate Patient Needs: Conduct thorough assessments by reviewing patient history, performing specific evaluations, and determining appropriate treatment plans. Communicate findings clearly to patients and their families to involve them in the goal-setting process.
• Implement Treatment Plans: Demonstrate clinical expertise by implementing tailored treatment protocols based on individual patient evaluations and their specific health needs, while adapting to changes in their conditions over time.
• Collaborate with Healthcare Team: Engage with other healthcare professionals to coordinate care effectively. Recognize and document any changes in patient conditions that warrant an alteration in treatment strategies.
• Manage Patient Documentation: Ensure precise and timely record-keeping for all patient interactions, which includes detailed initial evaluations, progress notes, and discharge summaries that reflect the patient's rehabilitation journey.
• Promote Accessibility to Care: Participate in initiatives aimed at removing barriers to access, utilizing community data to enhance services and engagement with the patient population.
• Engage in Continuous Learning: Take charge of your own professional development by pursuing ongoing education opportunities, staying informed on best practices in physical therapy, and contributing to departmental goals.

Patient Population / Client Focus

Qualifications

• Bachelor's degree in Physical Therapy from an accredited institution.
• Current license to practice as a Physical Therapist as per state regulations.
• American Heart Association's BLS for Healthcare Providers CPR certification.

• Strong clinical reasoning skills to assess and modify treatment plans as required.

Preferred Experience
