Key Responsibilities\n

• Conduct thorough assessments to identify and prioritize social, emotional, and environmental challenges that hinder clients' recovery.

• Develop detailed service plans that encompass specific goals, interventions, and regular progress monitoring to ensure clients receive the necessary support throughout their detox process.

• Provide one-on-one and family counseling, utilizing therapeutic techniques to help clients navigate emotional and psychological hurdles associated with substance use.

• Collaborate with clients to create personalized service plans, enhancing their engagement and commitment to recovery by connecting them with vital community resources and support systems.

• Offer immediate crisis intervention and risk assessment, developing safety plans to ensure clients are protected during vulnerable times.

• Coordinate with local agencies and community organizations to effectively advocate for the needs of clients and facilitate access to essential services.

• Engage in outreach efforts with the goal of raising awareness about the services offered by the Hartford Mental Healthcare Center and promoting community involvement.

• Participate actively in case conferences and team meetings, sharing insights and strategies to enhance overall service delivery.

Patient Population / Client Focus\n

Our primary focus is on individuals undergoing detoxification from substances, many of whom face complex social challenges such as homelessness, domestic violence, and poverty. This role requires a deep understanding of trauma-informed care principles and the ability to navigate the distressing situations that clients may face as they begin their journey toward recovery.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution.

• Current state licensure as a Licensed Clinical Social Worker (LCSW).

• Demonstrated knowledge of social and economic justice issues, as well as community resources relevant to client needs.

• Strong communication skills, cultural competence, and critical problem-solving abilities.

Compensation and Benefits\n

This role offers a competitive salary range of USD 60,000 to 100,000 annually, commensurate with experience and qualifications.
